Appointments to fit new employees can be coordinated by completing the on-line fitting request form.
Once the fitting request form has been received, the employee will receive a confirmation with the directions to the fitting location.
Uniforms will be sent to the attention of the clinic manager and will be received within 4 to 6 weeks.
Yes, Front Office employees can contact Cintas directly at 1-800-864-3676. Inform the representative that you are a UCLA employee. Back Office employees can order their own replacement uniforms by visiting smartscrubs.com/ucla and entering their employee ID to login. A valid credit card will is required to purchase your own replacement uniforms outside of the ones budgeted by the University.
Cintas will alter blouses and shirts for $4.95 each. (Short sleeve and 3/ 4 length sleeve). Scrub bottoms can be ordered in petite.

Yes, if the uniform is defective or in case of a significant size change. The clinic manager will need to complete the Cintas Quality Assurance Form and use the return label that is included with your order - follow the instructions. For Smartscrubs contact the Smartscrubs vendor directly at 510-673-6048 and use the return lable that is included with the order.
Scrubs are available in maternity sizes. Please visit the SmartScrubs website to view maternity options. Please note – Smart Scubs does not offer the olive green scubs for the MA's/HA's in maternity sizes. The Front Office uniform is not available in maternity sizes. Please contact Carmen James-Eggins (firstname.lastname@example.org) for maternity alternatives.
Uniforms are replaced by the University once a year. If you are eligibile to order replacement uniforms, go to our "Replacement Uniforms" page for more information.
UCLA uniforms are the property of the University and should be returned to the clinic manager on the last day of employment.
