For staff who have had their uniforms for over a year, the UCLA Faculty Practice Group has budgeted to replace a portion of the initial uniform order.
To help prolong the life of each employee sweater, please see the attached “Care for Sweater Guide”.
All replacement orders must go through the Ambulatory Operations uniform liason. Vendors will NOT accept orders directly from the clinic manager or employees. Please scan and email the completed request form to firstname.lastname@example.org or fax the completed form to 310-983-3039.

Front office staff who wish to purchase additional shirts/blouses or sweater may do so by contacting Cintas directly at 1-800-864-3676. Inform the representative that you are a UCLA employee. A valid credit card will be required in order to complete your order.
