Submitting an FPG Service Request.
1. Download and save the FPG Service Request Form
2. Complete the FPG Service Request Form and E-mail it to Property Manager, Gerardo Maciel at GMaciel@mednet.ucla.edu
3. Your request will be reviewed and you will receive a response within 1-2 business days.
4. Upon completion of the work, our Team Member will require the requesting manager's signature confirming completed work, duration and satisfaction.
In the event that we are unable to accommodate your request due to the degree of work necessary, we will work with our vendors to complete your service.
